The new health insurance premiums for 2026 have been announced.
The big question is not just which insurer to choose, but if you should take out supplementary insurance (aanvullende verzekering) at all. This depends entirely on how often you expect to visit the practice.
Many of our clients consciously choose specialized healthcare providers who work without contracts. But what does this mean for your reimbursement?

In addition to the big household names, this overview highlights a few specialized insurers (such as Stad Holland and ONVZ/VvAA). Especially if you value “free choice of provider,” these parties often offer the best conditions.

Scenario 1: The “Light User” (approx. 6 treatments per year)
Do you expect only occasional treatments in 2026 (e.g., once every 2 months or a short treatment plan of 6 sessions)? Pay close attention.
- The Math:
Supplementary insurance quickly costs between € 275 and € 350 per year.
Example: You visit the chiropractor 6 times (€ 65 per visit) = € 390.
However, many “budget” packages only reimburse up to € 250 per year or have a low daily cap. You would end up paying the premium plus extra costs out of pocket. - Advice for Chiropractic: Often do not insure. Putting the premium amount into a savings account is usually cheaper.
- Advice for Physiotherapy: With 6 physio sessions (approx. € 240 total cost), the premium is often higher than the actual costs. Paying out of pocket is cheaper.
Scenario 2: The “Average User” (approx. 9 to 12 treatments per year)
Do you need regular maintenance or do you expect to rehabilitate an injury? This is often the tipping point.
1. Focus on Chiropractic (Supplementary)
You are looking for a package with a reimbursement of at least € 350 to € 450.
- Recommendation: Stad Holland AV Standaard.
Why: Stad Holland works without contracts entirely and reimburses market rates. You don’t have to worry about “non-contracted care” penalties.
Coverage: € 350 pot for alternative care (note: max € 30 per day).
- Alternative: DSW AV-Standaard.
Why: Similar to Stad Holland, very reliable. Coverage € 450 (max € 30 per day).
2. Focus on Physiotherapy (Non-contracted)
You are looking for a package with 9 to 12 treatments.
- Recommendation: Zilveren Kruis 2 Sterren or CZ Plus.
Why: They offer 12 treatments for a mid-range price. Note: always check their rate list for non-contracted care.
3. The Best Combination (Chiro + Physio)
- Our Tip: VGZ Aanvullend Beter (or IZA Extra 1 for civil servants).
Coverage: 16x physio + € 300 alternative care.
Cost: You pay approximately € 340 premium per year. If you use both budgets, you will get more value back than you paid.
Scenario 3: The “Heavy User” (20+ treatments per year)
Do you have chronic complaints or do you play sports intensively? For patients who use a lot of non-contracted physio and chiropractic care, there are really only three “Kings of Free Choice” in 2026: ONVZ, Stad Holland, and DSW.

1. Focus on Chiropractic (Highest daily cap)
With many treatments, the reimbursement per day is crucial. If you pay € 65 per consult and the insurer only covers € 30 (like DSW/Stad Holland), you still pay a lot yourself.
- The Winner: ONVZ Optifit (also VvAA Optifit & PNOzorg).
Why: Daily reimbursement of € 65 (the highest in the market) and a yearly pot of € 550.
2. Focus on Physiotherapy (Non-contracted)
Many insurers only pay 50-70% if your physiotherapist does not have a contract. These parties do not do that:
- The Winner: Stad Holland AV Top or AV Extra.
Why: Stad Holland does not sign contracts with anyone. You simply get the market rate reimbursed at every physiotherapist. No fine print.
- Runner-Up: DSW AV-Top (27 treatments, very flexible policy).

- Choose Stad Holland if you need an extremely high amount of physiotherapy (36x) and want absolute certainty about free choice without contract hassles.
Tip: Special Collectives (IZA, VvAA, PNO)
Do you belong to a specific professional group? Then you can often benefit from the same high-quality coverage as the expensive brands, but with a discount or extras.
- Do you work in Healthcare? -> VvAA
- The VvAA health insurance is virtually identical to ONVZ (including that excellent € 65 daily cap for chiropractic and 30x physio in the Optifit package), but often slightly cheaper or with member benefits.
- Do you work for the Government/Municipality? -> IZA
- IZA (part of VGZ) has the IZA Extra 3 policy. This offers extensive physio coverage and often extra prevention budgets that standard VGZ policies do not have.
- Do you work in the Creative Sector / Media? -> PNO
- PNOzorg (part of ONVZ) offers the same strong conditions as ONVZ. Ideal for those who value free choice.
